We aim to make it quick and easy for you to find what you are looking for. Whether you need a medical or maternity exemption certificate or want to know what to do if you can't find a doctor who'll accept you as a patient - we're here to help.
Not sure if you need to go to your GP Practice for an NHS form or if it can be downloaded? Check the following:
Prescription Pre-payment Certificate Refund Application - You can download an online version of the Prescription Pre-payment Certificate Refund Form (PDF, 60kb). And send it in to your regional Practitioner Services office (see form guidance for details). Or alternatively obtain a paper form from your local Pharmacy.
You can obtain a exemption certificate application form from your GP Practice.
