The pharmacy will issue you with a HCS( R) prescription refund claim form. You should complete the form ensuring the appropriate parts of the form (parts E and F) are ticked and signed, then post the form back for the attention of:-

(A pre-paid envelope should have been issued by the pharmacy).
On receipt of the completed HCS( R) claim, your refund for prescription charges will be processed and a cheque will be posted out to you.
