Submit by email - complete online and submit electronically via NHSmail.
All forms must be sent from a dentist's personal NHSmail account, ie joebloggs@nhs.net

Forms 283, 286, 287 and 289 should be sent to nss.psd-customer-admin@nhs.net

State your list number and form type in the subject field of your email. If forms are submitted from any other email accounts, they will not be processed and you will be notified by email.

For assistant and trainee dentists, your employing dentist must submit all forms via their own personal NHSmail account.

If you do not have an NHSmail account, contact you local IT Facilitator.

If you have multiple forms to return, ie Dental 283 and 287, please attach these to separate emails.

Submit by post - complete the forms online, print off and return to us by post. Multiple pages should be stapled together.

Note: To enable these forms to work correctly, the minimum requirement is Adobe Reader, version 7.
