Item of service payments are defined in the
Statement of Dental Remuneration. This document defines the fees and criteria for payment claimed for treatment and services provided under the general dental services regulations.
Claims can be submitted
electronically
or on paper:
Electronic forms - through Electronic Data Interchange (EDI). For each treatment claim, practices are required to complete a GP17PR form, signed by the patient.
Paper forms - GP17 for general dental treatment or GP17(O) for orthodontic treatment. Supplies of GP17 and GP17(O) forms are available from your NHS Board.
