Forms Library

Allowance Forms are used to claim for various allowances, including Remote Areas Allowance, Rent Re-imbursement, Maternity Payments and Practice Improvement Grant.

Administration Forms are used for claim adjustments (Dental 283), non-payment enquiries (Dental 286) and patient detail amendments, including updating postcodes (Dental 287).

The schedule query form (Dental 289) should be submitted if you feel a patient has been incorrectly withdrawn, transferred, or moved onto reduced registration payments.

Prior approval forms are used for Discontinued Orthodontic Appliance Therapy (PSD 103), Orthodontic Appliance Interim Payment (PSD 264), Periodontal Charting (PSD 284) and Supporting Information (DPD 285).

Superannuation form, FC21, is for use under Regulation 3A12 and Schedule 1, paragraph 8 for determining contributions payable by, and in respect of, an assistant practitioner. This form can be completed online, but must be printed and signed before sending to us, at the address on the form.

To enable these forms to work correctly, the minimum requirement is Adobe Reader, version 7.
