GP21A forms
The GP21A forms are used by NHS Boards to request, resign or amend contractor details in our dental payment system, MIDAS.
The Scottish Government Health and Social Care Directorates issued a circular detailing the changes to the GP21A forms and process, effective 2 July 2010.
The GP21A forms must be completed online by NHS Boards only and submitted to us via NHSmail.
