Long Term Sickness Payments

What is Long Term Sickness Payment?

This allowance is paid to cover a period of sickness, where the dentist is unable to provide general dental services.  This takes effect after a period of 4 weeks, and will be paid to a maximum of 22 weeks, for any period of sickness.

How do I claim for Long Term Sickness Payment?

You can download the GP202 claim form or request it from your local NHS Board. The sickness certificate must be submitted along with the claim form.

Criteria:
1) Name included in dental list for at least 2 years; and
2) earnings of at least £27,800.00 or an NHS percentage of 90% or more.
