Superannuation form FC21
Superannuation form FC21 is for use under Regulation 3A12 and Schedule 1, paragraph 8 for determining contributions payable by, and in respect of, an assistant practitioner. This form can be completed online, but must be printed and signed before sending to us, at the address on the form.
