Quality & Outcomes Framework (QOF) - Forms

Application for Additional GP Practice User(s)

This form should be completed for every new GP Practice user to QOF Calculator and either faxed or posted to the appropriate Regional Office.

User accounts will be created by the QOF Calculator Support Desk and account details will be automatically e-mailed to the user(s).

Application for Additional NHS Board/CHP User(s)

This form should be completed for every new NHS Board / Community Health Partnership (CHP) user to QOF Calculator and either faxed or posted to the appropriate Regional Office.

User accounts will be created by the QOF Calculator Support Desk and account details will be automatically e-mailed to the user(s).

Application to Amend a User Account: All Organisation Levels

This form should be completed when amending a QOF Calculator user account. Practitioner Services has to be notified of changes in order to keep the system up-to-date.

This form should be either faxed or posted to the appropriate Regional Office. You will be notified by the QOF Calculator Support Desk once the change has been made.

Application for De-activation of User(s)

This form should be completed when a user is to be be removed from QOF Calculator. Practitioner Services has to be notified of changes in order to keep the system up-to-date.

This form should be either faxed or posted to the appropriate Regional Office. You will be notified by the QOF Calculator Support Desk once the change has been made.

QOF Interim Achievement/Aspiration Amendment Form

This form should be completed for each GP Practice whose achievement payment will not be confirmed in QOF Calculator by Friday 17th June 2011, and thus requires interim achievement and aspiration payments to be made.
