If you have not received payment for a particular claim after 3 months, submit an Ophthalmic 25 form, for the attention of customer administration.
If we are unable to trace the claim, you will be asked to submit a duplicate, along with a copy of the patient’s record card.
We regret we cannot accept duplicate GOS1, GOS3 or GOS4 claims after one year from the completion of treatment.
