Patient Detail Amendments

We hold a master record for each patient who has received NHS ophthalmic treatment in Scotland. This record has a unique identity number, which makes sure patient treatment histories are linked together.

Each month, the payment schedule report contains a section detailing the patients we have received a claim for. Although matched to the unique record of that patient, they are slightly different.

The section provides the patient details we hold, referred to as the master record, and the details entered on the claim.  If our master record is wrong, you can provide the correct details using the Ophthalmic 26 form. We will then update the details on our master record. This is referred to as a Patient Detail Amendment.

If the incorrect details are not amended, this could lead to patients not being matched correctly. This may cause treatment to be time-barred, as it may appear that the patient attended earlier than the time limit specified in the regulations.
