Practitioner Services - Enabling Primary Care

Practice Administration

You must make sure you are on the ophthalmic list of every NHS Board where you intend to provide general ophthalmic services (GOS), prior to commencing work.

Failure to comply with this means that no payment can be made for any work undertaken.

Counter signing is not allowed.

All application forms are provided by your NHS Board.

You must apply to join the list and wait for confirmation of acceptance onto the list from the NHS Board.

Practices are issued with a Payment Location number and every practitioner on a NHS Board list is allocated a Personal List number. This can be used in any area of Scotland, however, you must join the list of each Board you practice in.

We allocate the numbers on request from the NHS Boards.

If you wish to open a new ophthalmic practice, you need to contact the relevant NHS Board. For more information see New Practitioners.

If there is a Change in Practitioner’s Details or the bank mandate, you need to inform the NHS Board, and us, of the amendments.

To make a claim you will be required to complete the claim form, see Tips on Form Filling.

Keeping a note of claims

It is important that the full patient details are entered on the patient's record. The record card must be retained for a period of 7 years and we can request these records at anytime.
