Which HCS (R) Form should I issue to the patient for a prescription charge refund?

There are 2 different types of HCS(R) claim forms:-

HCS(R) PPC claim – is for a pre-payment certificate refund application form
HCS(R) prescription refund form - for refund of NHS prescription charges
